One of many clinical practice guidelines developed by Fraser Health Hospice Palliative Care Program. Provides recommendations for assessing and addressing psychosocial
issues of adult patients (age 19 years and older) who are facing a progressive life limiting
illness. It is designed to provide some general considerations and information about
psychosocial care, support and interventions to assist all health care providers within
their scope of practice and to enhance their engagement with patients and families.
